Copy of Pennsylvania Teaching Certification
Copy of undergraduate transcripts and graduate transcripts, if applicable.
PRAXIS Scores
School Employee Health Record. This must be completed by your physician and may not be more than one year old at the time of application. The TB test should be within the last three months.
Act 34 Report of Criminal History (original to be presented to Business Office). This report may not be more than one year old.
Act 151 PA Child Abuse History Clearance (original to be presented to Business Office). This report may not be more than one year old.
FBI Federal Criminal History Record (original to be presented to Business Office). This report may not be more than one year old.
Three letters of recommendation.

Application for Classified Employment
