Absentee Forms

When a student is absent from school, a written excuse card must be turned in to the office within 3 days of the absence. Forms can be obtained from the homeroom teacher or the main office.

Educational Trip Approval Forms

Students who are going to be absent from school due to a planned trip must request an education absence in order to have those days marked as "excused." The validity of the absence will be determined by the building principal. No educational absences will be granted during PSSA testing periods or during the first/last 10 days of school. The educational trip request form must be completed properly and returned to the office 10 days prior to the start of the leave. Days missed as part of an unapproved trip will be marked as "unlawful." Educational trip approval forms are in the office.

Media Release form

The Hamburg Area School District is proud of our students and wants to share news about activities, events and happenings that occur in the schools with parents/guardians and the community. This may be done through school publications, the district's web page on the Internet or local media including, but not limited to, television, radio or the newspapers. To respect your right to privacy, we are requesting that you notify the building principal in writing by completing the form linked to this section title if you have any objection to your child's name and/or photo appearing in such publications. The District will assume you consent unless you notify the building principal in writing.
