FREE Fall 2023 PIAA Sports Physicals offered by LVHN

Students who wish to participate in a sport for the 2023-2024 school year are required to complete a NEW physical prior to participation.

HASD's official sports medicine provider (Lehigh Valley Health Network) is offering families the opportunity to complete this mandate for FREE.

Families can choose to have the PIAA packet completed at their PCP. However, the date the physician signs the bottom right of SECTION 6 must be June 1 or later.  As such, if your annual well visit was prior to June 1, the packet will need to be returned to your PCP and the physician must date the packet after 6/1. The actual physical can have occured prior to 6/1.  However,  any date of "certification" prior to 6/1 will not be accepted by PIAA.

Packets for Fall sports are due in the athletic office mailbox at the high school by 8/1 for High School sports and 8/8 for Middle School Sports.  If your annual well visit is after those dates, you only have 3 options:

1) Go to the LVHN FREE opportunity

2) Go to a quick care facility

3) Return to your PCP and have the physician write a "certification" date of 6/1 or after on the bottom right of Section 6.
