Kindergarten Registration Information

  • Pennsylvania School Immunization & Examination Requirements

     

    FOR ATTENDANCE IN ALL GRADES CHILDREN NEED THE FOLLOWING IMMUNIZATIONS:

    4 doses of tetanus, diphtheria, and acellular pertussis* (1 dose on or after the 4th birthday)

    4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)**

    2 doses of measles, mumps, rubella*** (1st dose on or after the 1st birthday)

    3 doses of hepatitis B

    2 doses of varicella (chickenpox) vaccine or evidence of immunity (1st dose on or after the 1st birthday)

    *Usually given as DTP or DTaP or if medically advisable, DT or Td

    **A fourth dose is not necessary if the third dose was administered at age 4 years or older and at least 6 months after the previous dose

    ***Usually given as MMR


    ON THE FIRST DAY OF SCHOOL, unless the child has a medical or religious/philosophical exemption, a child must have at least one dose of the above vaccinations or risk exclusion.

    • If a child does not have all the doses listed above, needs additional doses, and the next dose is medically appropriate, the child must receive that dose within the first five days of school or risk exclusion.  If the next dose is not the final dose of the series, the child must also provide a medical plan within the first five days of school for obtaining the required immunizations or risk exclusion.
    • If a child does not have all the doses listed above, needs additional doses, and the next dose is not medically appropriate, the child must provide a medical plan within the first five days of school for obtaining the required immunizations or risk exclusion.
    • The medical plan must be followed or risk exclusion.

    Children ENTERING SCHOOL (at Kindergarten or 1st Grade) need the following examinations:

    Physical examination and Dental examination

    Examinations are acceptable if done no sooner than one year prior to the beginning of the school term your child will be entering.  Private physical examination and private dental examination forms are available from the Health Forms section.  The forms are to be completed by your child's physician and dentist. If you do not have internet access, forms can be obtained from your school nurse.

    *It is recommended that your child’s doctor and dentist perform the required examinations since they can provide the appropriate treatment and follow-up.  School examinations will be offered, but are screenings only and should not replace regular examinations by your child’s doctor and dentist.  

     

    Additional Thoughts:

    Kindergarten children often are in need of a change of clothing during the school day due to spills or not getting to the bathroom on time.  Please consider sending in an extra shirt, underpants, and pants to school in a bag marked with your child’s name that can be kept in the Health Room for only your child’s use.  Children are more comfortable changing into their own clothing.  We do not want to interrupt your busy day at home or work unnecessarily.

    Please work with your child over the Summer months to attain independence in the bathroom.
