Unless there is an extreme extenuating circumstance, we will not accept paperwork after the submission deadlines for Fall 2025.

DEADLINE TO SUBMIT PHYSICAL PAPERWORK: 8/1/25

Requests to submit paperwork late must be made in writing and sent to aarmen@hasdhawks.org prior to 7/22/25. Work and vacation will not be considered acceptable reasons to submit late paperwork. Mandatory attendance for Fall sports begins:

          8/4/25 HS football

          8/11/25 All other HS Sports & MS Football

          8/18/25 All other MS Sports

Participants are required to complete the PIAA physical process prior to participation.  Any PIAA paperwork you submitted for the 2024-2025 school year is invalid.

YOU ARE PERMITTED TO TAKE THE PAPERWORK TO ANY DOCTOR IN PENNSYLVANIA (family, quick care, etc)

• Participants who wish to take the paperwork to their family doctor are welcomed to do so.  Please be sure the Certification date on the bottom right of page 6 is not prior to 5/1/25, or PIAA will not accept it.
