Contractor/Vendor Clearance Requirements

All Contractors/Vendors working for the District must provide copies of their Child Abuse Report, PA Criminal History Report, the FBI Federal Criminal History Report, and the Act 24 Arrest/Conviction Certification Form. These documents must be received by Human Resources prior to doing work within a District building or on District grounds. All clearances must be less than 60 months old and must be for employment purposes. Volunteer clearances cannot be accepted.
Each individual who will be on site to perform work must submit his/her background check packet either electronically or via email:
- Contractors/Vendors who have an ongoing work relationship with HASD - please click here to submit your background clearances electronically.
- Contractors/Vendors performing a one-time service may email a complete background check packet to the Director of Human Resources. Please submit one PDF file titled "Lastname, Firstname" containing all 4 documents.
A. Act 151 PA Child Abuse History
- Click the link above to start the process.
- Click on “Create Individual Account.”
- You must create a “Keystone ID” and follow the step-by-step directions for receiving a temporary password by email, and then creating a new password. Be sure to keep your Keystone ID as you will need this to log back into your account to create a new password.
- Be sure to request this clearance for employment purposes. You will need a credit card for payment.
- When results are available, you will receive an email. Log back into your account and view the official results. Save the results page to your computer and also print a copy for your records.
- You will receive a copy of your clearance in the mail which can take up to two weeks.

- DO NOT SUBMIT A COPY OF YOUR RECEIPT. YOU MUST SUBMIT THE CERTIFICATION FORM ITSELF.
B. Act 34 PA State Police Criminal History Report
- Click the link above to start the process.
- Click “Submit a New Record Check (requires a credit card)”
- Follow step-by-step directions on the site. Submit for employment purposes.
- Results will be available online once you complete the process.
- When results are ready, click the Control number to save/print a registration receipt – this is not the official State Police Clearance. Please click the “CERTIFICATION FORM” link which appears below the record check details.
- Save this certificate to your computer as a PDF and print a copy for your records.
- DO NOT SUBMIT A COPY OF YOUR RECEIPT. YOU MUST SUBMIT THE CERTIFICATION FORM ITSELF.
C. Act 114 FBI Federal Criminal History Report (Online registration through IdentoGo) (The Service Code for Pennsylvania PDE School Districts is:1KG6XN)
- This is a two-step process: (1) Register online (click the link above) and schedule your fingerprinting appointment; (2) Visit a fingerprinting site to have your fingerprints submitted electronically.
- Please be sure to bring an appropriate form of identification with you to your appointment. Payment for the clearance is required at the fingerprinting location.
- After your fingerprints have been recorded, you will receive an unofficial copy of the report. You must provide the unofficial copy of the report in your packet and the report must contain a UEID number starting with "UZSV". Clearances submitted through the Department of Human Services cannot be verified and will not be accepted.
- DO NOT SUBMIT YOUR RECEIPT. YOU MUST SUBMIT THE UNOFFICIAL COPY OF THE FBI REPORT.
D. PDE-6004 Act 24 Arrest/Conviction Report & Certification Form
- Please read the instructions on this form carefully. This document must be signed and recently dated (if this document is older than 30 days it will not be accepted).
- If you are submitting your contractor/vendor application online, you will receive an auto-generated email seeking completion of this form. If you do not receive this email within a few minutes of submitting your application materials, please check your SPAM/Junk folder. Otherwise, you may download this form by clicking on the "PDE-6004 Act 24" link above.
