The School Nurses can be contacted at your child's school Monday through Friday from 8:15 a.m. to 3:45 p.m.

IMMUNIZATION INFORMATION
FOR ATTENDANCE IN ALL GRADES CHILDREN NEED THE FOLLOWING IMMUNIZATIONS:
4 doses of tetanus, diphtheria, and acellular pertussis* (1 dose on or after the 4th birthday)
4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)**
2 doses of measles, mumps, rubella*** (1st dose on or after the 1st birthday)
3 doses of hepatitis B
2 doses of varicella (chickenpox) vaccine or evidence of immunity (1st dose on or after the 1st birthday)
*Usually given as DTP or DTaP or if medically advisable, DT or Td
**A fourth dose is not necessary if the third dose was administered at age 4 years or older and at least 6 months after the previous dose
***Usually given as MMR
ON THE FIRST DAY OF SCHOOL, unless the child has a medical or religious/philosophical exemption, a child must have at least one dose of the above vaccinations or risk exclusion.
- If a child does not have all the doses listed above, needs additional doses, and the next dose is medically appropriate, the child must receive that dose within the first five days of school or risk exclusion. If the next dose is not the final dose of the series, the child must also provide a medical plan within the first five days of school for obtaining the required immunizations or risk exclusion.

- The medical plan must be followed or risk exclusion.
If you have any questions about immunization compliance, please contact the nurse at your student's assigned school.
PHYSICAL AND DENTAL EXAMINATIONS
By Pennsylvania State Law, children are required to have a physical examination in Kindergarten or 1st grade, students entering from out of state, or new students without record of having a physical examination in these required grades. *
By Pennsylvania State Law, children are required to have a dental examination in Kindergarten or 1st grade, 3rd grade, students entering from out of state, or new students without record of having a dental examination in these required grades. *
Examinations are acceptable if done within one year prior to the beginning of the school year your child will be entering.
Documentation of a private examination needs to be provided to the school nurse to fulfill this requirement.
*It is recommended that your child’s doctor and dentist perform the required examinations since they can provide the appropriate treatment and follow-up. School examinations will be offered, but they are ONLY screenings and should not replace regular examinations by your child’s doctor and dentist.

PENNSYLVANIA SCHOOL HEALTH SCREENINGS
Height and Weight - All students are weighed and measured yearly. BMI (body
mass index) is calculated. Parents will receive a confidential letter with this information.Vision - Vision screenings are provided each year to children in Kindergarten through 5th grade. If your child does not pass his/her vision screening, a referral form will be sent home with the student. Your child should be examined by an eye physician for further evaluation and treatment, if needed. School vision screenings do not take the place of a thorough eye examination by an eye doctor. Please make sure your child has a full eye exam annually or as recommended by your health care provider.
Hearing – Hearing screenings are provided each year to children in Kindergarten, and children in 1st through 3rd grade. Screenings are also provided with teacher or parent requests. If your child does not pass his/her hearing screening, a referral form will be sent home with the student for further evaluation and treatment, if needed.

Medical Plan Card (Red and White Card)
-If a child does not have all of the doses of the PA School Vaccination Requirements, needs additional doses, and the next dose is medically appropriate, the child must receive that dose within the first five days of school or risk exclusion.
