IMMUNIZATION INFORMATION
The Hamburg Area Middle School is served by a full-time school nurse. It is the school nurse’s duty to see that the requirements, as outlined in the school health laws, are enforced. In sixth grade, it is mandated by the state that each student receives a physical examination either by his or her family physician or by the school doctor. We recommend that the exam be done by your family physician, since he/she can best evaluate your child’s health. The physical may be obtained over the summer before sixth grade or during the school year. If you choose not to go to your family doctor, with your permission, the physical will be given by our school doctor at no charge. The private examination is done at the parent’s expense. Forms for this exam are available at the HASD website and from the school nurse. If you get the physical in the summer, please mail the form to the middle school nurse or drop it off in the school office.
Below are the Pennsylvania Department of Health's immunization requirements for students attending 7th grade:
- 1 dose of tetanus, diphtheria, acellular pertussis (Tdap) by the first day of 7th grade
- 1 dose of meningococcal conjugate vaccine (MCV) by the first day of 7th grade
ON THE FIRST DAY OF 7TH GRADE, unless the child has a medical or religious/philosophical exemption, a child must have the above vaccines or risk exclusion from school.
Please note: There will be no provisional enrollment period. These immunizations will be required in order to attend the first day of 7th grade. Students who are not immunized in accordance with the Pennsylvania State regulations, or who do not qualify for medical or religious exemptions to immunizations, must be excluded from school.
The Tdap vaccine is a booster to be given to adolescents. It is distributed under the names Adacel and Boostrix. Please do not confuse this vaccine with the adult DT. If at least five years have elapsed since your child’s last tetanus and diphtheria immunization, your child would have to be immunized with the Tdap vaccine. The meningococcal conjugate (meningitis) vaccine is distributed under the brand name, Menactra. You may want to discuss these immunizations with your child’s physician during their mandated 6th grade physical.
FOR ATTENDANCE IN ALL GRADES CHILDREN NEED THE FOLLOWING IMMUNIZATIONS:
4 doses of tetanus, diphtheria, and acellular pertussis* (1 dose on or after the 4th birthday)
4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)**
2 doses of measles, mumps, rubella*** (1st dose on or after the 1st birthday)
3 doses of hepatitis B
2 doses of varicella (chickenpox) vaccine or evidence of immunity (1st dose on or after the 1st birthday)
*Usually given as DTP or DTaP or if medically advisable, DT or Td
**A fourth dose is not necessary if the third dose was administered at age 4 years or older and at least 6 months after the previous dose
***Usually given as MMR
ON THE FIRST DAY OF SCHOOL, unless the child has a medical or religious/philosophical exemption, a child must have at least one dose of the above vaccinations or risk exclusion.
- If a child does not have all the doses listed above, needs additional doses, and the next dose is medically appropriate, the child must receive that dose within the first five days of school or risk exclusion. If the next dose is not the final dose of the series, the child must also provide a medical plan within the first five days of school for obtaining the required immunizations or risk exclusion.
- If a child does not have all the doses listed above, needs additional doses, and the next dose is not medically appropriate, the child must provide a medical plan within the first five days of school for obtaining the required immunizations or risk exclusion.
- The medical plan must be followed or risk exclusion.
PHYSICAL AND DENTAL EXAMINATIONS
Children in 6th grade are required to have a physical examination.*
Children in 7th grade are required to have a dental examination.*
Examinations are acceptable if done no sooner than one year prior to the beginning of the school term your child will be entering.
*It is recommended that your child’s doctor and dentist perform the required examinations since they can provide the appropriate treatment and follow-up. School examinations will be offered, but they are ONLY screenings and should not replace regular examinations by your child’s doctor and dentist.
The school nurses do the growth, vision, hearing, and scoliosis screenings throughout the school year.
